+ Nurse Leader
Delta Health needs a Nurse Leader for our Puerto Rico Telehealth division. The successful candidate will oversee and manage the utilization of technology and services delivered by our Registered Nurses. We are looking for a highly professional RN who is comfortable leading a clinical team and coordinating activities to ensure compassionate, effective and efficient delivery of high-quality clinical care to patients that we serve. A Nurse leader will be responsible for identifying new patients that qualify for our services and serves as the gatekeeper to our patient population list. A Nurse Leader will also collaborate with the operations management team in communicating our key operational targets and leading the team to achieve the periodic goals set by leadership.
Skills you’ll bring & what you’ll do:
- Manage Teams of RNs and APPN
- Maintains professional and technical knowledge by attending educational workshops and participating in professional societies
- Maintains patient confidence and protects operations by keeping information confidential
- Maintains continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs
- Documents patient care services by charting in patient and department records
- Resolves patient problems and needs by utilizing multidisciplinary team strategies
- Assures quality of care by adhering to therapeutic standards by measuring healthcare outcomes against patient care goals and hospital or regulatory standards
- Identifies patient care requirements by establishing personal rapport with patients and their families
- Counsel patients and family members before, during and after treatment
- Respond to inquiries, concerns and complaints from patients and their parents or guardians
- Create and contribute to patients’ medical records as needed
- Consider the age of the patient and his or her emotional stability when determining appropriate treatment
- Collaborate with physicians and multidisciplinary team members to create a Plan of Care for each patient
- Communicate with physicians, LPNs, radiologists and other medical professionals

+ Informatics Nurse
Delta Health needs a Registered Nurse (RN) for our Puerto Rico Telehealth division. The successful candidate will enjoy utilizing technology to deliver healthcare solutions with a focus on geriatric patients with chronic conditions. We are looking for a highly professional RN who is comfortable providing care remotely and communicating with doctors as well as tending to the needs of his or her patients. We offer a positive, upbeat work environment where all medical personnel work together for the good of the patient.
